Dr. Lori Osso-Connor has been working in the area of hospice and palliative care since 2002. She received both her bachelor degree and doctorate degree in pharmacy from Duquesne University in Pittsburgh, PA in 1995 and 2002 respectively. She received her board certification in geriatric pharmacy in 2014. Dr. Connor has been a clinical pharmacist in such practice setting as retail pharmacy, medication therapy management consultant, and hospice and palliative care consultant. She has been with Delta Care Rx since 2012 as a hospice clinical pharmacist.

After graduating from Duquesne University with her doctor of pharmacy degree in 2013, Dr. Sydney Janusey spent most of her time working in managed care pharmacy and building a clinical skill set. She also has experience with specialty medications and is interested in behavioral health. Dr. Janusey joined Delta Care RX as a hospice clinical pharmacist in the fall of 2015.

Dr. Holly Lassila has been a hospice clinical pharmacist for the past 9 years. She is an Associate Professor of Clinical, Social and Administrative Sciences at the Mylan School of Pharmacy at Duquesne University in Pittsburgh, PA and received her Pharmacy and Doctor of Public Health degrees from the University of Pittsburgh in Pittsburgh, PA. In addition to her hospice work, Dr. Lassila works as a clinical pharmacist with the underserved population in Pittsburgh, PA. She received her MSEd in Community Mental Health Counseling from Duquesne University in 2013 and specializes in trauma recovery. She joined Delta Care RX in 2013.

Dr. Jessica Horsley earned her Doctor of Pharmacy degree from University of Charleston in Charleston, WV. She joined Delta Care Rx as a Hospice Clinical Pharmacist in 2014 and brings experience from hospital, home infusion, and mental health specialty pharmacy practice settings. Dr. Horsley is an active member of the American Society for Health-System Pharmacists and associated state organizations and has presented research regarding medication reconciliation at the national meeting, as well as provided student programming support for state meetings. Her background as a musician gives her knowledge and perspective on successful non-pharmacological interventions in the treatment of the hospice patient. Dr. Horsley plans to achieve board certification in 2017.

Dr. Joseph Thomas is a 2013 graduate from Duquesne University Mylan School of Pharmacy. He successfully completed a concentration in the inaugural Pharmaceutical Entrepreneurship program at Duquesne University. Dr. Thomas spent time working as a community pharmacy manager until joining the Delta Care Rx team as a clinical pharmacist in 2015.

Dr. Shane Donnelly is a PGY2 Pain and Palliative Care Pharmacy Resident at Delta Care Rx. He received his doctorate of pharmacy degree with a specialty certificate in Medication Therapy Management from Duquesne University in May of 2015. Dr. Donnelly’s areas of focus in residency training with Delta Care Rx include palliative therapeutics, pain management, academia, and infectious diseases. He started his professional career with Delta Care Rx as the PGY1 Pharmacy Practice Resident in June of 2015.

Dr. Irene Petrides received her doctorate in pharmacy from Duquesne University in 2008 with a concentration in geriatric pharmacotherapy. Upon graduation, Dr. Petrides practiced in a community pharmacy as the Pharmacist Manager at CVS Pharmacy in Concord, North Carolina. She joined Delta Care in 2013 as a hospice clinical pharmacist.

Michelle Mikus
Dr. Michelle Mikus graduated with her doctorate of pharmacy from the Honors College at Duquesne University in Pittsburgh, Pennsylvania. While in college at Duquesne, she worked in retail pharmacy for 5 years, providing her with a background that has made her both patient oriented and efficient. Dr. Mikus has completed the Heart Failure Certification course through the University of Southern Indiana as well as the Education in Palliative and End-of-Life Care (EPEC) program through the Feinberg School of Medicine at Northwestern University. Dr. Mikus has been on the Delta Care Rx team since September 2013 as a hospice clinical pharmacist and is the pharmacy manager of ProCure Pharmacy.
